All you need to know about registering for accessibility services at AU
Once you’ve identified the accommodations and services you need, you will need to complete and submit the Verification of Accommodation (VOA) form. This form has two essential components.
Let’s dive into Sections 1 and 2 of the VOA form a little more:
Section 1: Student information and self-assessment
First, you’ll begin a self-evaluation of your needs. Here you will be able to freely describe any past experiences and functional impacts you expect to face while completing course activities. This allows us to create an accommodation plan that is tailored to your specific needs.
Section 2: Professional verification
This section refers to the medical verification provided by a health practitioner on their letterhead. They must describe the impact on academic activities you experience due to a disability.
Complete the Verification of Accommodation (VOA) form and return it to the Accessibility Services department via email or fax.

Why do we need both self-assessment and medical verification?
The insights you share with us in Section 1 of your application are critical for our team to understand your anticipated needs and create a relevant accommodation plan for studying at AU. It clearly describes your experiences and helps us recommend just the right services.
The documentation we receive from your doctor in Section 2 of your application serves 2 important purposes:
- It confirms that you are a person with a permanent or temporary disability.
- It provides us with information about the severity and the impacts you may encounter and recommended accommodations.
